Medicaid for Braces in New York State Explained [Video Guide]

Medicaid for Braces in Buffalo, NY | Free Consults [Video Guide]

If you’re on Medicaid and you’re considering a smile correction for your child, you probably have a few questions about coverage. At Orthodontists Associates of Western New York, we accept most insurance plans for orthodontic treatment, including Medicaid for braces. However, Medicaid plans in particular limit coverage to very specific cases of medically serious orthodontic issues for patients under the age of 21. To receive coverage, one of our orthodontists must submit records to Medicaid demonstrating that your child’s issues are severe enough to qualify. If their case meets the threshold and Medicaid approves it, you should have very little to no out-of-pocket expense for your child’s orthodontic treatment.

Our team understands how much patients depend on insurance to afford care. We want you to have clear expectations about how your insurance will apply to your child’s treatment. That’s why we don’t charge until treatment begins. When you choose our practice, it costs nothing to learn how Medicaid applies to your child’s orthodontic correction.

Medicaid for Braces in New York: Coverage Details

Medicaid for Braces in Buffalo, NY | Free Consults [Video Guide]For Medicaid to cover braces in New York, you need to meet very specific criteria. Coverage only applies to patients under 21 with medically serious orthodontic problems. The New York Medicaid orthodontic benefit is for children and teens with severe malocclusions, which are bad bites that significantly affect health or function.

Medicaid in NY will only approve braces if an orthodontist can show they’re medically necessary. This is typically when the orthodontic problem interferes with basic functions like chewing, speaking, or breathing. Medicaid could cover your child’s braces if they’re part of a comprehensive cleft palate treatment. Other examples where your child’s braces might be covered are:

  • If the malocclusion results in impacted permanent front teeth
  • If an accident, injury, or disease has damaged the jaws or teeth so badly that it has changed the way the teeth fit together
  • If an extreme overbite, overjet, or crowding makes it difficult to bite or chew

In contrast, mild to moderate crowding, small overbites, spacing, or appearance‑only concerns usually do not qualify, even if they bother your child. Clear aligner treatment is usually not covered when braces would suffice. Lastly, Medicaid does not offer a routine braces benefit for adults over 21.

Medicaid for Braces Scoring System

New York uses a standardized scoring system, the Handicapping Labio‑Lingual Deviation Index (HLD), to determine whether a case is severe enough for coverage. Severe conditions, such as very large overjets and overbites, impacted teeth, or major skeletal discrepancies, automatically qualify according to the HLD. Other cases are scored by severity and must meet a high enough score to qualify.

An orthodontist at one of our practices will examine your child and produce orthodontic records. Then, they will submit a detailed case with the HLD score to Medicaid or the managed care plan. If your child doesn’t present a situation that automatically qualifies, they need to go through the numeric scoring system. With this process, they receive points based on how many millimeters the bite is off. This is based on the specific measurements of any overjet, overbite, open bite, crowding, and spacing issues that your child presents.

When approved, Medicaid should cover your comprehensive orthodontic treatment, often at no cost to you. This includes braces, routine adjustments, and standard retainers within the approved treatment period.
